About this service

If you are the donor in a lasting power of attorney (LPA), whether property and financial affairs or health and welfare, and you wish to object to your LPA being registered, you should use this form to raise your objections to the Public Guardian. After sending this form, the registration of your LPA will be suspended by the Office of the Public Guardian until a decision is made. You should send this completed form directly to the Office of the Public Guardian.
